As part of the application process, you will need to go to Eat Breathe Thrive's application page, where you will be directed to two further forms: a Participant Informed Consent and a Liability/Media Release. In order for your application to be considered, all three forms must be completed and submitted. The entire application process will take about 10-15 minutes in total.
Once we have received your complete application materials, you will be notified by email of your application status within 5-8 business days. At that time, we will give you instructions on how to register for your program.
Notes: in the "location" option, please put "Bay Area". This is a "community program"
